Vaginectomy is the surgical closure or removal of the vaginal canal. It is an optional procedure — not all FTM patients choose vaginectomy — but for many it is a deeply meaningful step in eliminating dysphoria associated with the presence of a vaginal opening. It is typically performed as part of a surgical stage alongside hysterectomy, metoidioplasty, or phalloplasty — rather than as an isolated procedure.
Vaginectomy involves removing the vaginal mucosa (lining) and closing the vaginal canal in layers. The resulting perineum has a masculine appearance. Vaginectomy is required before urethral lengthening to the perineum in phalloplasty, as the vaginal tissue is used in the reconstruction.

Is vaginectomy required for phalloplasty?
Vaginectomy is not an absolute requirement for phalloplasty, but it is strongly recommended when urethral lengthening is planned — as the vaginal mucosa is used in the urethral construction and having an open vaginal canal in combination with a constructed urethra increases complication risk. Dr. Bhavya Sree will advise on the optimal plan for your specific case.
Can I still have a Pap smear / cervical screening after vaginectomy?
If the cervix remains (i.e. hysterectomy has not been performed), cervical screening may still be possible through the perineum with a specialised approach, though it is more complex. Most FTM patients planning vaginectomy also have a hysterectomy — eliminating the need for cervical screening. This is discussed as part of your overall surgical planning.
